Blitz Build “2008” Volunteer  Application            
September 6-12 and 22-26

Thanks for your interest in the Habitat Detroit Blitz Build 2008. Accepted volunteers agree to work a minimum of 1 day of the Blitz.

Important instructions for completing this application:
1. Please read this application and answer each question thoroughly. For additional information, please visit our web site at:

2. A “Release and Waiver of Liability” must be signed and brought with you on the first day of volunteering.
    Volunteers  must be at least 16 years old to participate. Teens 16 & 17 must be accompanied by an adult supervisor to volunteer

3. We must receive this completed and signed application with your payment of $50.00 no later than TUESDAY,   
    September 2, 2008.

PARTICIPATION FEE: $50.00
(This fee Includes registration, breakfast & lunch daily, basic tool kit needed for the Build ($35 value) & t-shirt)
  

Please note that all volunteers associated with house or hospitality sponsors (e.g Ford or Carlson or Dow, Christ Church, Grosse Pointe Partners, etc.) do not have to pay the fee. Please indicate below that you are part of a sponsor group.

In addition for all designated house & crew leaders, the fee is waived. The house and crew leaders have been predetermined. If you are a house or crew leader, please choose the “sponsor” category and indicate “house/crew leader” for your group if you are not associated with a sponsor.
